Transcript: Pediatric Genitourinary Trauma Dr. Javanmard Dr. Nicknama Urology department Shohadaye Tajrish Hospital Renal trauma Pediatric Renal Trauma -Flank pain -Gross microscopic hematuria Presentation presentation: Note: absence of hematuria alone does not rule out significant renal trauma in children. physical examination: -CVA Tenderness of flank -Ecchymosis -Hypotension -Tachycardia Physical examination Note: in children, sympathetic tone may sustain normal blood pressure longer than in adults, even with significant blood loss, making hypotension a less reliable sign for significant blood loss in renal trauma Mechanism Mechanism of injury is an important predictor of renal trauma in children: -Rapid deceleration -High velocity injury Note: Other organ system injuries are present in about 50% of cases of pediatric blunt renal trauma including liver, spleen, head, or orthopedic injuries. Imaging Studies for Diagnosis Imaging Fast FAST The FAST examination has low sensitivity in diagnosing renal trauma injuries because it is performed in the emergency department, often by emergency department providers, and is usually utilized to look for free fluid in dependent regions of the abdomen and does not specifically evaluate for organ injuries. When the FAST examination is combined with serial physical examinations for a 24-hour period, a normal FAST examination and normal physical examinations over 24 hours is very good at ruling out any clinically significant renal or intraabdominal injuries. Dedicated kidney sonograph KIDNEY SONOGRAPHY ultrasonography can be used for the initial evaluation of renal trauma in children, but CT is preferred. Abdominopelvic CT scan CT SCAN For hemodynamically stable patients, CT is currently the most sensitive and specific radiologic test to diagnose and rule out genitourinary truma. AUA and EAU trauma guidelines recommend obtaining an IV contrast-enhanced abdominal and pelvic CT scan with immediate and delayed images for the evaluation of renal injury In children, a single contrast phase abdominal and pelvic CT scan, because of a lack of knowledge about recommended imaging. The obvious limitation of this is that the accuracy in diagnosing urine extravasation and isolated ureteral injuries is decreased, is The EAU urogenital trauma guidelines acknowledge that practically, the initial CT scan is often a single phase, and additional phases can be obtained if renal injury is not characterized well enough (often obtained to limit radiation exposure.) High risk criteria Management Some specific findings for grade 4 to 5 injuries on CT scan are associated with subsequent intervention. Medial extravasation of contrast is worrisome for possible ureteropelvic junction disruption and is associated with intervention. Lack of contrast seen in the ureter in the setting of grade 4 injury is associated with subsequent intervention for urine leak in pediatric series. If greater than 25% of renal parenchyma is devitalized, this has been associated with a significantly higher rate of angioembolization. intervention has been associated with the number of these risk factors present. with 1 or fewer risk factors having less than 10% intervention risk and two to three risk factors having greater than 50% risk for interventionand open surgery. Nonoperative Management Protocol The first steps in nonoperative management are to confirm lack of hemodynamic instability and grade the renal injury Manegment Serial HB checking :CBR continued until gross hematuria has resolved and blood counts have remained stable. The duration of bed rest and intensity of observation varies by injury severity.
